On your first visit:

The initial consultation includes the following steps and may take 20-40 minutes:

• Examination according to TCM (Traditional Chinese Medicine) and Western Medicine.
• Analysis of your body imbalances according to TCM and Western Medicine.
• Treatment Plan developed for Acupuncture and Chinese Herbal Formulas
• Suggestions for diet, exercise and stress reduction.

​Before your first visit, you can fill out our form and bring it with you on your initial visit. You can down load the initial form PDF format now. Please fill it out and bring it to Dr. Chen when you come for the first visit.

If you are from another state or would like to have a long distance consultation, please fill out the form and fax it to Dr. Chen's office at 212-564-3732. Make an appointment by the phone or webcam consultation with Dr. Chen by calling 212-564-3324.  Please prepare all the information that you have regarding Blood Tests, X-rays, MRIs, Ultrasound, medical reports, etc before the phone or webcam consultation.

If you have insurance (in the accepted insurance list), it is better to contact us before you come. We can help you to check your insurance benefit and coverage. You have to provide the followings information:

1. Your full name.
2. Your date of birth.
3. Your insurance company’s name and number ID. (on your insurance card).
4. Your insurance provider’s phone number (on your insurance card).
5. Your contact phone number.

We will let you know how many visits your insurance company allows, how much your co-pay and deductable will be, etc.
