If you are from another state or would like to have a long distance consultation and herbal formula treatment, please fill out the form and fax it to Dr. Chen's office at 212-564-3732. Make an appointment for the phone or webcam consultation with Dr. Chen by calling 212-564-3324. .Please prepare all the information that you have regarding Blood Tests, X-rays, MRIs, Ultrasound, Medical Reports, etc. before the phone or webcam consultation.

Dr. Chen will prepare a special herbal formula or herbal supplement for you based on your symptoms and saturation and then will mail it to you. You can pay the phone consultation fee, herbal formula fee, and delivery fee by credit card or personal check.

Usually we follow up on long distance patients every two weeks or month by phone or webcam. According to the patient's situation and response to the herbs, Dr. Chen will adjust the formula and dosage.
